  12.  entry form below.  Send completed form to CompuServe 70154,1577,
  13.  (Internet 70154.1577@compuserve.com) or regular mail (diskette).
  14.  We will acknowledge email submissions sent to us via CompuServe.

  16.  DISTRIBUTION:  WWIS  is distributed as  WWIS.ZIP  on  CompuServe
  17.  (GO UKSHARE or GO ASPFORUM), and our free BBS at (514) 345-8654.

  19.  form,  and may charge the recipient up to  $9 U.S.  to cover the

  59.  GUIDE TO DATA ENTRY:  DO NOT ALTER THE FORMAT OF THE FORM.  When
  60.  submitting  your form,  you need to send  only the lines between
  61.  (and including) the  FIRST LINE and FINAL LINE  separator lines.

  63.  (1)  Place an "X" in the appropriate boxes. If this is the FIRST
  64.       TIME you have  submitted a listing,  mark  New Listing.  If
  65.       you are CHANGING a listing,  check off Update if it is MORE
  66.       than a year old,  or Early Update if it is LESS than a year
  67.       old  ($5 fee for Early Update).   If the mailing address is
  68.       already cited in  another listing, also place an "X" beside
  69.       Extra (there is a $5 fee for each extra listing).
  70.  (3)  Enter the name of your company, if any.
  71.  (4)  Enter  your name  here or  (for a company)  the name of the
  72.       primary contact at your company.
  73.  (7)  Enter up to four phone numbers, with descriptions, such as:
  74.       Voice line, Fax, BBS (9600 bps), Order line, etc.
  75.  (8)  Enter your online addresses.   Use  "Other"  to specify the
  76.       name and ID for other services, such as Internet, Fido etc.
  77.  (9)  Describe your top five  services or products.  If a product
  78.       is for computers other than PC compatibles, please say so.
  79.  (10) Write up to  four  lines  about the person or company being
  80.       described.  You can mention such things as: number of years
  81.       in shareware, important lessons learned, and so on.
  82.  (11) If you have provided any  special service  to  promote  the
  83.       shareware industry,  describe  your  contribution.  You can
  84.       list your affiliations (e.g. ASP, STAR).   Also, list which
  85.       shareware conferences you participate in more than once per
  86.       week (e.g. CompuServe ASP or UKSHARE, GEnie HOSB, etc.).
